The various forms required for SNF access are described on this page. If your organization or research group already has an account/project on file with SNF, and you simply want to add a new user to your exising account, see <below>. If your organization/research group wants to establish a new account/project, the forms required are determined by your labmember category:

Services Administrator Forms may also be emailed as attachments to faciliate registration, but we will require signed paper forms for our files before your registration can be completed. Please allow at least three working days to process registration. Follow-up with Labmember Services Administrator to ensure all forms have been received and are complete. Project Proposal Form: All new Projects require this form to be filled out. (Labmembers working on existing Projects do not need to fill out this form). This form should serve as the basis for your discussion of your new Project with the appropriate Technical Liaison. For a more detailed explanation of Projects, click <here>. Project Proposal forms are reviewed solely for feasibility (does SNF have the resources to meet your project needs?) and materials compatibility (to ensure against cross-contamination of others' work). SNF does not review proposals for scientific merit (as your funding agency has already done this!) User Information Form: This form, to be filled out by each Labmember, gives us your contact and billing information and must be filled out each time there is a change in either of these. Agreement Form: This disclaimer form states our policies and conditions. It must be signed and submitted by every non-Stanford Labmember, including Remote Users, before accounts are issued and work commences. Please read this carefully before signing and submitting. CIS Grant Form: The industrial sponsors of the Stanford CIS (Center for Integrated Systems) have established the User Grants program to encourage new, non-Stanford, academic researchers to join the SNF community. These grants average about $5K, which is generally sufficient for at least a couple of months' worth of lab fees. CIS grants are awarded by project, to researchers with PI standing (generally, faculty) in any U.S. college or university, other than Stanford. Details about the terms of the award are described on the application form. The CIS Grant form should be submitted with the Project Proposal. |
